Your First Visit
This is your chance to meet the Doctor. She/he will ask you several questions during the consultation (medical history review) time to determine if your symptoms or complaints will benefit from our services. In order to maximize your time with the Doctor, we ask that you come prepared.
Below is a list of items to bring on your first visit:
- Photo ID
- Insurance Card or Information (if using insurance)
- Co-Payment*, if applicable
- Completed Registration Form (download and complete)
- Recent x-rays (if taken within one year of appointment date)
- Any questions you may have for the doctor
*Check with your insurance company for your co-payment amount. We will collect $25 at the time of your visit until we verify insurance coverage.
Our goal is to reduce your symptoms and improve your functional health. In order to do so, we have to be a team. The Doctor will create a customized treatment plan specifically designed for you. Although the Doctor has goals she/he would like to see you reach, it is important that you have some health goals of your own. Therefore, as a team, we will work together to getting you back on the Road of Recovery. It is highly important that you adhere to the treatment plan. Each visit builds upon the next and failure to stick to your treatment plan may result in: worsening of symptoms, a decrease in quality of life, a sense of hopelessness or depression, chronicity, and more. If at any time you would like to discontinue treatment, just let the Doctor know. Remember, we are here to help you the best way that we can.
